Responsibilities

Serve as primary point of contact for small practices by working closely with physicians and their staff to implement process and systems improvements that lead to improved quality of care delivery and health outcomes.

  • Provide technical assistance to physicians to help them improve the health outcomes of patients, particularly smoking, cholesterol, hypertension, and aspirin-prophylaxis.
  • Provide support for office redesign (e.g., workflows, documentation, standard processes) to improve office efficiency.
  • Provide additional coaching on preventive-health features of the EMR and how to use them as part of QI activities
  • Provide a forum for discussing performance feedback and sharing best practices for QI efforts
  • Liaison between small practice providers and various units with the PCIP to provide feedback on quality improvement efforts
Qualifications Required
  • RN or PA with at least 2-3 years of clinical experience preferred
  • Outpatient internal medicine/primary care experience preferred
  • Demonstrated knowledge of principles related to quality improvement (including NCQA measures), health care statistics as well as automated data information systems.
  • Advanced skills in data management as well as the ability to analyze and present data to groups and individuals
  • Ability to develop, organize, and facilitate special projects
  • Ability to work collaboratively with physicians, staff and external organizations
  • Strong computer skills
  • Willingness to travel within the 5 boroughs

The NYC Department of Health and Mental Hygiene (DOHMH), Primary Care Information Project (PCIP) Taskforce is an interdivisional program designed to use health information technology to improve population health in NYC. As part of a Mayoral priority, the Taskforce coordinates the DOHMH's activities to provide leadership in citywide initiatives to extend the use of electronic health records (EHR) among thousands of ambulatory primary care providers. The DOHMH will help to transform the delivery of health care services to medically underserved communities throughout the City by helping providers to move from a paper based to an electronic health care delivery system. DOHMH anticipates that this unprecedented $60 million project will fundamentally improve access to information, thereby assisting providers to delivery higher quality care.

In collaboration with The Fund for Public Health in New York (FPHNY) , a not for profit corporation serving as program partner with DOHMH to implement and advance public health initiatives, PCIP is seeking Quality Improvement Consultants. Under contract to FPHNY, the Consultant will collaborate with DOHMH senior Quality Management staff to implement a systematic, practice-facing program for health outcomes management and quality improvement efforts.

Qualifications Required

We are Camp Lokanda, one of the top camps on the eastern coast on the United States. We are in upstate New York, set in the amazing Catskill Mountains, on Lake Loch Ada, from which we named our camp. We have been around for more than 70 years. We are a seven week co-ed traditional sleep-away camp, and have about 450 campers, ranging from 6 to 16.

We operate a very modern and comfortable infirmary. We have a doctor in camp 24 hours a day on constant call. We have at least 2 nurses and a infirmary receptionist. We have a day patient and overnight patient seperation. All our infirmary rooms are air conditioned to make our campers stay there more comfortable. We have the support of a local hosptial and specialists in the surrounding area for anything more than the usual camp issues.

Lokanda prides itself on being a safe, fun environment for everyone to grow and develop. We only take the best staff to look after our campers. As you take good care of our campers, we in turn take good care of you. With a good salary and travel package, and lots of support and staff facilities.
